Our Palliative Care service provides compassionate care and support for people living with a life-limiting illness, as well as their families and carers. We work alongside you to support comfort, quality of life and your individual wishes throughout your palliative and end-of-life journey.

Our aim is to help people live as comfortably and meaningfully as possible, and to support them to receive care and spend their final days in the environment of their choice.

Our service includes:

• Symptom management and comfort care
• Support with advance care planning and end-of-life decision making
• 24-hour on-call nursing support, 7 days a week
• Collaboration with local GPs and Nurse Practitioners to support medical care
• Emotional and bereavement support
• Access to a range of other services and supports to meet individual needs
